Home Medication Reviews

Have you ever heard of a home medication review?  They are particular helpful for people on multiple medications and can be arranged by asking a GP for a referral.  A UFS pharmacist then visits the home to discuss the current medications a person is taking.  The pharmacist is looking to see if medications are taken regularly, if any are out of date, what the person does and doesn't understand about them and any contraindications between medicines.  This is a free service funded by Medicare.
