Home Medication Reviews (HMR) aim to enhance the quality use of your medicine and reduce adverse effects. These in-home reviews are particularly helpful if you are on multiple medications. The review includes a comprehensive medication review conducted by an accredited pharmacist in your home. These can be arranged by asking your GP for a HMR referral.
A specially accredited UFS pharmacist then visits your home to discuss the current medications you are taking. The pharmacist will check if medications are taken regularly, if any are out of date, anything you may not understand about your medications and whether there are any contraindications between medicines.
This is a free service funded by Medicare.
